Hagler was the first child of Robert Sims and Ida Mae Hagler, born on May 23, 1954. He was raised by his mother in the Central Ward of Newark, New Jersey, along with five siblings: sisters Veronica, Cheryl, Genarra, and Noreen, and half brother Robbie Sims, who also became a professional boxer. He first put on gloves at age ten, taught by a social worker he knew only as Mister Joe. Hagler dropped out of school at 14 and worked in a toy factory to help support the family. Following the 1967 Newark riots, which destroyed the family tenement and left 26 people killed and $11 million in property damage, his family moved to Brockton, Massachusetts. Hagler began boxing after a street fight with local boxer Dornell Wigfall, who beat him and stole his jacket. He trained at a gym owned by brothers Pat and Goody Petronelli, who became his trainers and managers. To enter amateur tournaments, he lied about his age, claiming he was born in 1952 instead of 1954. In May 1973, he won the National AAU 165-pound title after defeating Terry Dobbs, and was voted 'Outstanding Boxer' of the tournament. He turned professional with a 55–1 amateur record.

He is an inductee of the International Boxing Hall of Fame and the World Boxing Hall of Fame, was twice named Fighter of the Year by The Ring magazine and the Boxing Writers Association of America, and was named Fighter of the Decade (1980s) by Boxing Illustrated. Many analysts consider him to have one of the most durable chins in boxing history; he was knocked down only once, in a debatable knockdown by Juan Roldán of Argentina.
